Open System Preferences and go to Keyboard:

Select the Shortcuts tab. Make sure the box for "Use keyboard navigation to move focus between controls" is checked.

Open Safari Preferences:

Select the Advanced tab. Make sure the box for "Press Tab to highlight each item on a webpage" is checked.

Please complete the History Form and bring it to your appointment along with the Medical Eligibility Form. Make sure the form is signed by the parent/guardian. Only the Medical Eligibility Form gets uploaded to your AthleticClearance.com account. Forms can also be picked up in the main office.
